The development of frailty services is a priority for the Trust, both in theacute siteand incommunityservices. We are part of the Acute Frailty Network in 2020, and are looking to increase the geriatricianinput at the front door of the hospital, and in same day emergency care, whilst also being part of the admission avoidance andcommunity offer. North Yorkshire has an ageing population, with 24.2% of the population in 2018 being over the age of 65. Projections indicate that the population in the over 85 age group is expected to increase in Harrogate by approximately 30% by 2025.HDFTprovide bothacute and community services, so in conjunction with our partners, weare in a great position to strengthen our frailty pathways from the front door of the hospitalto our patients homes.

You will be joining a dynamic and enthusiastic team of Consultant geriatricians withcomplementary skills, and with special interests in acute care, community rehabilitation, movement disorders, stroke medicine, orthogeriatrics, continence, dementia, falls and polypharmacy. With the addition of these newposts we will be able to provide a daily frailty post-take ward round on weekdays, and strengthen our input into the expanding Supported Discharge Service and Community Care teams, promoting a 'home first' ethos. We are supported by junior doctors and advanced clinical practitioners, as well as a diverse and highly valuedmultidisciplinary team, who share our passion for delivering high quality person-centred care. You would have the opportunity within your job plan to develop your own interests.

As an acute frailty geriatrician you will also work closely with colleagues inacute and general medicine, andin theEmergency Department. The organisation of our acuteservices is under constant review in the context of COVID-19 but we have shown resilience, flexibility and compassion in the way that we have providedacute services.

We are a provider of undergraduate medical school education for the University of Leeds and have a thriving research community. This is an exciting opportunity for a new, or experienced geriatrician to join our team!
