We are seeking a Community Associate Specialist (Elderly Care) and a Consultant Community Physician to join our friendly home treatment teams in rural west Kent.

You’ll be part of a team dedicated to adult and elderly community healthcare, actively managing patients living with frailty and other complex conditions in their homes and hospitals; and working closely with clinical, social and external partners.

You’ll provide expert support to our innovative Home Treatment Service and oversight of medical care for community hospital inpatients. You will support the continued development of the Home First service, contributing to strategic decision making. You’ll also provide leadership and mentorship of specialty doctors and advanced nurse practitioners (ANPs)

Based in idyllic Tonbridge in west Kent, this role involves travel around one of the most beautiful parts of the country.

You’ll need to be flexible, creative and ready to embrace new ways of working as we continue to invest heavily in the community sector. In return we’ll offer you a rewarding post in a relaxed and welcoming environment that gives you the opportunity to expand your knowledge and skills.

Although this is a full time post, candidates wanting to work less than full time or flexibly are welcomed.
